Alan is getting some x-rays done at the moment and after this he have to make an appointment to see the doctor to see whats the result and the after that another MRI.
Patient has completed his 6 months treatment for pulmonary TB. Trying to get an appointment with Pulmonologist for repeat Xray as advised before.
He has improved appetite and eating well.
His right vision seem to have improved based on observations. He is still able to somehow recognize colors. He is able to recognize faces of family members although unable to remember only a few of their names
He is aware of his surroundings. His reflexes are okay. In terms of physical strength - he is still able to a few walks every morning, occasionally he loses his balance , still able grip and lift light to medium objects.
Experienced a few headaches recently but goes away after a whiile - this could be due to extreme hot weather.
There are no new changes in his prognosis. Just monitoring his condition on a daily basis until the next MRI is completed.
Patient is scheduled to do another MRI by May to check for any changes in the size of the tumor

Update
First Check up with Oncologist was completed regarding the result of MRi following radiation session.
After the MRI what is the prognosis for Allan?
Results show that there is no change or increase in the size of the tumor. According to the Doctor he is happy about this since it appears that the tumor is not really that active and may still be benign and this is very good. Based on his assessment, the patient appears to be okay, and responded to the radiation treatment very well. He can converse, his right eye sight appears to have slightly improved and although with guidance patient can move and walk normally, with no side effects observed.
What was the cause of the edema?
The presence of edema is normal given that he received radiation and the enlarged ventricles is due to this.
What will be the treatment for the edema and the enlarged brain ventricles?
None. It is expected to subside eventually. In some cases steroids may be given if there is an effect on patient such as pain or weakness. But given the patient's condition Doctor doesn't see the need for it
What is the next step? Are further radiation or chemo envisaged for the tumor?
No, he does not suggest any at this time. To continue monitoring the patient., MRI after 3 months (May) then can do another after 6 months ( Nov). Patient is being referred back for assessment also by Dr.Gimenez. and Neuro Ophtha. Recommends physical activity for the patient.
Comparison is made based on the most recent MRI of the brain dated 08/13/23. On the diffusion weighted images, no acute infarct is identified. Again demonstrated are extensive post -operative changes related to the prior left pterional craniotomy. Again seen is the known large heterogeneously-enhancing presumably calcified residual left sphenoid wing meningioma. It appears to be unchanged since the last examination. It measures 7.0 x 6.0 x 6.3 cm in AP, transverse and craniocaudal dimensions compared with remeasured dimensions of 7.0 x 5.9 x 6.4 cm on prior study. It is again noted to completely encase the left internal carotid artery, as well as the proximal left middle cerebral artery. It has likewise infiltrated the sellar and the suprasellar compartments, as well as the left cavernous sinus, unchanged. Ti causes the same degree of moderate compression of the left lateral aspect of the midbrain. There is progressive surrounding vasogenic edena particularly aling the left inferior frontal region abd across the callosal genu. Therebis a persistent entrapment of the lateral ventricle, particularly the left temporal horn. The overall degree of ventricular dilatation has mildly progressed since the last examination. The posterior fissa structures demonstrate no nee abnormal findings. The orbits are normal. There is mucosal thickening involving the left maxillary sinus.
Alan finished his MRI today and will get the result tomorrow. Will update tomorrow.
Alan is continuing his medication for pulmonary TB. He was shifted from Fixcom4 to Fixcom2 for four more months as part of and to complete his treatment. There was evidence of improved appetite .
Before the radiation treatment was initiated it was noted that he mentioned everything was blurred and couldn't see the smile on people's faces anymore...he can only see the body form and recognize the gender by voice. Now, his right vision seem to have improved a little based on observations. He is showing signs he can see some details on the faces of people around him.
and he is now more alert and aware of his surroundings. Alan is scheduled for MRI on Feb 4 after which follow up check up with his Radiation Oncologist / Neuro Surgeon
The last day of radiation had finished. But in the course of the treatment Alan has suffered a setback. He had accumulated fluids in his lungs and have to under go an operation to drain the fluids out. Also the test showed he have tuberculosis. After that he had his last radiation after a week of treating him. He have to wait now for 3 months to have an MRI, to see if the tumour in his head decrease. His appetite have decrease dramatically and he is not eating well at all, lost a lot of weight.
Alan’s last radiation finished last Nov. 14, the next step is to help him regain any body weight loss. He was referred to nutritionist.
Last week Alan accumulated fluids in his lungs which has to be drained straight away. 3 litres of water was taken out, today his 29th radiation was just done.
