Hi I am Manish Narayan I am here to ask for help from all you lovely people in my this difficult time.My father is having heart problem and needs urgent treatment which I can not afford on my own . I am working as a pastry chef in Rotorua for the last few years. my father had an open heart surgery back in 1998 which was financed by his work insurance now he is retired and is not obliged for the service.
My parents come to visit me in march, after spending a month my father started having problem breathing at night time. I took him to GP and he suggested to see the heart specialist and the letter is down below.

Thank you for referring this man who presented to you recently with a history of having had coronary bypass surgery in
1998 and for a while now he has been experiencing a distention of his abdomen after eating and a painful sensation going
up to the front of his chest and may be in his shoulders. He didn’t seem to get these symptoms with walking.
You put him on atenolol 50 mg/d and Isosorbide Mononitrate and he has been feeling a bit bitter since then.
He also complains of pains in his legs, initially he told me it was in the right leg along the line of vein graft harvesting but
when we put him on the treadmill today he had pains in both calves.
He was very very tired on the treadmill and in fact he had to stop at just under 2 minutes and he had quite deep ST
segment depression. ST segment depression was present on the resting ECG as well.
His blood pressure did not drop, it went up to 204/74 and resting blood pressure was 184/88.
So this man looks as if he has redeveloped obstruction either of grafts or native coronary vessels, he needs a coronary
angiogram and needs to be on aspirin.
I have given him a script for a nitrolingual spray and it seems that he has benefited from the atenolol and Isosorbide you
prescribed.
Obviously money is a problem to be able to afford further investigations let alone the treatment but I suggested that an
angiogram be done and it is hopeful that we may be able to help him by arranging an instalment plan for the fee.
His treadmill test was very positive and so I think he has got a severe problem.
